ADMISSION REQUIREMENTS

Age Requirement

• Nursery - 3 years to 3 years & 11 mos.
• Pre-Kinder - 4 years to 4 years & 11 mos.
• Kinder - 5 years to 5 years & 11 mos.

Note: Cut-off date of age requirement is dependent on school opening date.

Requirements for Application

1. APPLICATION FORM

2. Two (2) Recommendation Forms - Kindly send the link to your child's previous:
   (1) Class Adviser/Subject Teacher AND
   (2) Principal OR Guidance Counselor
   • Recommendation Form
3. 1x1 size most recent photo (white background) *
4. Photocopy of Current Report Card
5. 1x1 size most recent photo (white background) *
6. Photocopy of PSA Birth Certificate *
7. Photocopy of Baptismal/Dedication Certificate *
8. Photocopy of ECCD Checklist (for incoming Grade 1 students only)
9. Original Certificate of Good Moral Character/Certificate of Good Moral Character with ESC ID Number
10. Original Medical Certificate (issued by a doctor)
11. Photocopy of Immunization Record (if available)
12. Photocopy of COVID-19 Vaccination Card (if applicable)

PROCEDURE FOR ADMISSION

1. Fill out the Application Form and prepare to upload the admission requirements in PDF or JPG format.
   • Admission Form
2. Kindly wait for three (3) to five (5) days to process and evaluate your application. Qualified applicants will receive the Notice for Enrollment and Probation Contract.

ENROLLMENT PROCEDURE

Venue:

• Divine Light Academy, LP – Manuela Subdivision., Las Piñas City
1. Fill out the Application Form and prepare to upload the enrollment requirements.
• Enrollment Form
2. For new students, Endorsement Form from the Guidance Office will be routed to the Registrar's Office and Accounting Office.
3. The Accounting Office will email the enrollees for the Enrollment Assessment details and payment procedures.
4. After payment, send your proof of payment to the appropriate email address.

5. Kindly wait for a payment acknowledgement from the Accounting Office and an enrollment status update from the Registrar's Office.

PAYMENT SCHEMES

1. Annual payment – corresponding rebates are given.
2. Semi-annual payment – corresponding rebates are given.
3. Installment mode

Tuition & other required fees are posted on school bulletin boards during enrollment period.

CHOICE OF SESSION (Morning or Afternoon) will be strictly on a FIRST-COME FIRST-SERVED basis.
